Thighplasty 101: Reshaping Your Lower Body

What Is Thighplasty — and Is It Right for You?

Thighplasty is a surgical procedure that removes excess skin and fat from the upper thighs to create smoother, firmer, better-proportioned legs.

Quick facts:

  • What it is: A body-contouring surgery, also called a thigh lift
  • Who it’s for: People with sagging thigh skin from weight loss, aging, or genetics
  • What it fixes: Loose skin, chafing, rashes, and poor thigh contour
  • What it doesn’t do: It is not a fat-removal procedure — that’s liposuction
  • Recovery: Most patients return to a desk job in 2–3 weeks; full results visible around 6 months
  • Common combinations: Liposuction, tummy tuck, body lift

Over 9,000 thigh lift procedures were performed in the United States in 2020 alone — and that number continues to grow. For many people in New Jersey and beyond, no amount of diet or exercise can tighten skin that has lost its elasticity. Whether it’s from significant weight loss or simply the natural effects of aging and gravity, sagging thigh skin can affect how you move, how you feel, and how clothes fit.

Thighplasty addresses what exercise simply cannot.

I’m Dr. Valerie J. Ablaza, M.D., a board-certified plastic surgeon and partner at The Plastic Surgery Group of New Jersey, with experience in body-contouring procedures including thighplasty. In this guide, I’ll walk you through everything you need to know to make a confident, informed decision.

Understanding Thighplasty: More Than Just Fat Removal

When people think about improving their legs, they often think of fat removal. However, thighplasty is fundamentally different from liposuction. While liposuction is excellent for removing stubborn fat in patients with firm, elastic skin, it doesn’t address the “hanging” or “crepey” skin that often follows significant weight loss or the natural aging process.

According to the American Society of Plastic Surgeons, a thigh lift reshapes the thighs by reducing excess skin and fat. This results in smoother skin and better-proportioned contours for the lower body. At our practice, we view body contouring procedures as a way to “tailor” the skin to fit your new, smaller body after weight loss.

A good candidate for thighplasty generally meets these criteria as of April 2026:

  • Weight Stability: You should be at a stable weight for at least 12 months.
  • Non-smoker: Smoking significantly increases the risk of wound healing issues.
  • Overall Health: Managed medical conditions, such as diabetes, are essential for safety.
  • Realistic Expectations: Understanding that while the contour is vastly improved, surgical scars are a necessary trade-off.

Beyond the aesthetic benefits, there is a major functional component. Many of our New Jersey patients seek this surgery for functional improvement—to find relief from chronic chafing, rashes, and hygiene issues caused by skin-on-skin friction.

Choosing the Right Technique for Your Goals

No two pairs of legs are the same, which is why we offer several different thigh lift techniques. The choice depends on where your excess skin is located and how much needs to be removed.

The American Society of Bariatric Plastic Surgeons notes that different types are chosen based on the extent of tissue laxity.

TechniquePrimary Target AreaIncision LocationBest For
Mini Thigh LiftUpper third of the inner thighGroin crease onlyMild skin laxity
Inner (Medial) LiftFull length of the inner thighGroin extending to the kneeModerate to severe laxity
Outer (Lateral) LiftOuter thigh and hipGroin around to the back/hipSagging on the outside of the leg
Spiral LiftFront, back, inner, and outer thighCircular/wraparoundTotal leg transformation post-massive weight loss

Medial Thighplasty

The medial thigh lift is the most common version of this procedure. It targets the inner thigh, where skin is naturally thinner and more prone to losing elasticity. For patients who have undergone body contouring after weight loss, this technique is a game-changer. By removing the “extra” tissue that causes friction, we can significantly reduce skin irritation and improve the patient’s ability to walk and exercise comfortably. You can learn more about our specific thigh lift services to see how we customize this for each patient.

Lateral and Spiral Thighplasty

For those with sagging on the outside of the thighs or the buttocks, a lateral lift is more appropriate. This is often performed as part of a larger body lift or belt lipectomy. A spiral lift is the most comprehensive, addressing 360 degrees of the thigh to tighten the hips, outer thighs, and inner thighs simultaneously. This is typically reserved for our massive weight loss patients who require extensive contouring.

The Surgical Journey: Preparation and Procedure

Preparing for surgery is just as important as the surgery itself. When you visit us in Montclair or Livingston, we begin with a comprehensive body procedure consultation.

Preparation steps include:

  1. Medical Clearance: A full check-up to ensure your heart and lungs are ready for anesthesia.
  2. Smoking Cessation: You must stop smoking several weeks before and after surgery.
  3. Medication Adjustments: Stopping blood thinners or certain supplements that increase bleeding.
  4. Pre-operative Markings: On the day of surgery, we mark your skin while you are standing. This allows us to see exactly how gravity affects your tissue, ensuring the most accurate “tuck.”

Steps of a Thighplasty Procedure

A typical thighplasty takes between 2 to 5 hours, depending on the complexity.

  • Anesthesia: Most procedures are performed under general anesthesia for your comfort.
  • Incision: Based on the technique chosen, incisions are made. We make every effort to place these in natural folds, like the groin crease, where they can be hidden by swimwear.
  • Liposuction-Assisted: Research shows that liposuction-assisted thighplasty can reduce complication rates by nearly half. We often use liposuction to “thin” the fat layer before removing the skin, which preserves vital lymphatic vessels.
  • Tissue Excision and Anchoring: After removing the excess, we use deep sutures to anchor the tissue to the Colles fascia (deep tissue in the groin). This prevents the scar from “migrating” or pulling downward later.
  • Closing: We use a combination of deep sutures, skin adhesives, and tapes to ensure the most seamless post-weight loss body contouring result.

Recovery, Risks, and Long-Term Results

Recovery requires patience. You’ve spent a long time getting to this point, so don’t rush the healing process!

The Recovery Timeline:

  • Days 1-3: You will need to rest with your legs elevated. Most patients use surgical drains to prevent fluid buildup (seromas).
  • Week 1-2: Drains are typically removed. You can return to a “sitting” job.
  • Week 6: You can stop wearing your medical compression garments. These are vital for reducing swelling and helping the skin adhere to the new contours.
  • Month 6: Swelling is mostly gone, and you can see your final results.

It is important to be transparent about risks. Thighplasty has a higher complication rate than some other cosmetic surgeries—around 42% according to some studies. However, most of these are minor.

  • Wound Dehiscence: Occurs in about 51% of cases (small areas of the incision opening), which usually heal with basic wound care.
  • Seroma: Fluid buildup occurs in about 25% of cases.
  • Infection: Seen in about 16% of cases.
  • VTE (Blood Clots): A rare but serious risk (1%), which we mitigate with compression boots and early walking.

Knowing what a body lift includes and following all post-op instructions is the best way to minimize these risks.

Frequently Asked Questions about Thigh Lifts

Where will my scars be located and how will they look?

Scars are the “price” of the procedure. For a medial lift, the scar usually sits in the groin crease. If you have significant skin laxity extending to the knee, a vertical scar along the inseam of the leg is necessary. While scars are prominent in the first few months, they flatten and lighten over time. We recommend topical scar treatments and strict sun protection to help them fade.

Can I combine this with other procedures?

Absolutely. Many of our patients choose to combine a thigh lift with a tummy tuck or a body lift to address multiple areas in one session. Combining procedures means one anesthesia session and one recovery period, though the initial recovery is more intense.

What are the costs and will insurance cover it?

Thighplasty is usually considered a cosmetic procedure. However, if you suffer from chronic, documented skin infections or rashes that haven’t responded to medical treatment, some insurance providers may offer partial coverage. We can help provide the necessary medical documentation for your claim.
